Dr. Lee and the rest of our team always strive to make your care as simple and smooth as possible from beginning to end. That’s why we provide our new patient forms online. Instead of spending lots of tedious time in our waiting room, you can print them out at home and fill in all the pertinent information on your own schedule before your appointment even begins. Just remember to e-mail them to us or bring them with you to Muskingum Valley Oral Surgery for our front desk team!
Insurance
Dr. Lee is happy to accept most insurance plans. We can even file all necessary paperwork on your behalf so that you’re sure to receive the maximum amount of benefits available to you through your plan. We’ll always do what we can to ensure that your dental care at our office is as affordable as possible.

Payment Options
Payment for your dental care will be required once we’ve provided service. 50% of the required payment will be collected when the patient is using their insurance benefits. We accept cash, check, and most major credit cards. If you’re in need of a payment plan, our office has a partnership with CareCredit. This partnership allows our patients to accumulate no interest when on one of their plans, as long as they pay off the balance as scheduled.
